The process of categorizing renal cysts may be challenging, involving a series of decisions that may affect the final diagnosis and clinical outcome such as surgical management. Bosniak IIF cysts, which showed reclassification to the Bosniak III/IV category during radiological followup (12%), showed malignancy in 85%, comparable to Bosniak IV cysts. The estimated surgical number needed to treat to avoid metastatic disease of Bosniak III and IV cysts was 140 and 40, respectively. The Bosniak classification categorizes renal cysts into five groups. Category I. Benign simple cyst with thin wall without septa, calcifications, or solid components, and has a density of 0–20 Hounsfield units (HU) (about equal to that of water). In such cases, a CT scan without intravenous contrast is enough for classification.

The aim of this prospective study was to compare the classification of complex cystic renal lesions using the Bosniak classification with CEUS and MRI to CT. Forty-eight patients with 65 cystic renal lesions were included in this study. All participants signed written informed consent. 2021-02-22 Assessment of Incidentally Detected Simple Renal Cyst in Pediatric Patient Based on the Modified Bosniak Classification —Incidentally Detected Simple Renal Cyst in Pediatric. well-defined thin (≤2 mm) smooth wall; homogeneous simple fluid (-9 to 20 HU) no septa or calcification; the wall may enhance after administration of contrast; Class II .
Using Bosniak classification version 2019, cystic renal masses were classified into 12 category I, 19 category II, 13 category IIF, four category III, and 20 category IV by CT and eight category I, 15 category II, 23 category IIF, nine category III, and 13 category IV by MRI.

The Bosniak Classification, version 2019 (a) formally incorporates MRI into the classification, (b) includes specific definitions for individual imaging features and Bosniak classes, (c) incorporates a larger proportion of renal masses encountered in clinical practice (eg, incompletely characterized but highly likely benign cysts), and (d

This CT-based classification was introduced in 1986 and originally divided cystic renal masses into one of four classes after exclusion of infectious, inflammatory, and vascular etiologies ( Table 1) (1). Since then, refinements have reduced the number of benign masses in Bosniak class III (2–9).

Bosniak IIF: Requires follow-up. Needs ultrasound/CT/MRI follow up - no strict rules on the time frame but reasonable at 6 months. Bosniak III: Partial nephrectomy or radiofrequency ablation in elderly or poor surgical candidates.
